One woman's needs may be two to four times higher than anothers. Curiously enough, it is often the woman with a thin, lean build who needs greater amounts of oestrogen because she has very little production of oestrogen from her fatty tissues. Furthermore, it is the lean fine-boned woman who has a greater risk of osteoporosis and will need an adequate amount of oestrogen to prevent this. Modern-day HRT has become so safe and effective in the prevention of many problems that every menopausal woman should be considered a candidate for it. If no medical risks exist, HRT should be offered as an informed option. Notwithstanding this, it is still true that menopausal women differ in their need for HRT. Some cannot live without it saying that they feel ancient, decrepit and lifeless without it, while others hardly seem to notice when their ovaries finally stop producing oestrogen.
A minority of menopausal and post-menopausal women actually feels worse on HRT even after all different forms and dosages have been tried. Such was the story of Mandy, a 55-year-old big boned somewhat obese woman who complained of gaining 8kg in weight, fluid retention, aching legs and fatigue since beginning HRT three years ago. Her blood pressure was raised and her legs were bulging with varicose veins. Mandy had been started on hormone tablets at the time of her natural menopause without any blood tests or bone mineral density check being done to assess her individual need and dosage of oestrogen. I checked her bone mineral density and found it to be excellent at over 100%, she had no risk factors for osteoporosis; in particular, her mother had lived to 98 years of age with no trace of osteoporosis or cardiovascular disease.
Thus, it was not essential for Mandy to take HRT and indeed in her case, its side-effects were greater than its benefit. She stopped HRT and six months later had lost 6kg in weight and regained her normal blood pressure and generally felt much better. All Mandy needed was a doctor's permission to stop HRT under the assurance that her bones and blood vessels would be regularly checked to make sure that she would not increase her risk of osteoporosis and cardiovascular disease by not taking oestrogen. In my opinion, Mandy would probably do better without HRT.
Menopausal women should not be made to feel they must take HRT or terrible things will happen, but rather given an informed option as the merits of HRT vary between individuals.
