- Denomination.
Levitra
- Active substance.
Vardenafil
- Indications for Use.
Erectile dysfunction (failure to reach and retain erection, necessary for sexual intercourse).
- Action.
The drug is indicated for the treatment of erectile dysfunction, being an inhibitor of phosphodiesterase of type 5 (PDE5).
Erection of penis is the process based on the relaxation of smooth muscles of cavernous bodies and arterioles which are in them. From the nervous endings of cavernous bodies during sexual stimulation nitrogen oxide evolves (NO), which activates a ferment of guanylate cyclase, which provides the increase of cyclic guanosin monophosphate (cGMP) content in cavernous bodies. It results in relaxing of smooth muscles of cavernous bodies which favours the increase of blood reflux into penis.
Vardenafil blocks PDE5, under the influence of which splitting of cGMP occurs, it results in enhancing of performance of endogen NO in cavernous bodies during sexual stimulation, which determines the capability of Levitra to enhance a natural response to sexual stimulation.
- Posology and Administration.
By mouth, regardless of food intake.
In the beginning of treatment the recommended dose is 10 mg. (about 25–60 min. before a sexual contact). But it has been shown that Levitra is also effective if one takes it 4–5 hours before sexual activity. Maximal frequency of intake is once in 24 hours. Depending on efficiency and tolerance of the treatment a dose may be increased up to 20 mg. or decreased to 5 mg. in 24 hours. Maximal recommended dose is 20 mg. once a day. To provide an adequate response to the treatment a sexual stimulation is needed.
Patients of advanced age (above 65). The starting dose is 5 mg., because in this age category a clearance of Vardenafil is decreased.
- Counterindications.
– simultaneous therapy of nitrates and the drugs which are donators of nitrate oxide;
– childhood up to 16 years;
– elevated sensitivity to the drug’s components.
- Should Be Used with Caution If…
Should be used with caution in patients with anatomic deformation of penis (contortion, cavernous fibrosis or Peironi disease), likewise in patients with the diseases predisposing to the development of priapism (sicle-cell disease, multiple myeloma or leukaemia).
The safety of Levitra has not been studied yet, and the drug is not recommended for the use in patients with severe disruptions of liver functions; with kidney diseases in terminal stage, requesting haemodialysis; with arterial hypotension (systolic tension at rest less than 90 mm. of mercury column); recently had insult or myocardial infarction (within the last 6 months); unstable stenocardia; with hereditary degenerative diseases of retina (for instance, with pigmented retinitis).
As Levitra has not been used in patients with predisposition to bleeding and in patients with attack of ulcer disease, in these cases the drug administration is possible only after a thorough evaluation of equilibration of risk and usefulness of the therapy.
In investigations with a simultaneous use of Vardenafil as a monotherapy - changes in the bleeding time are not observed.
In connection that sexual activity always connected to a certain degree of cardinal risk, it is necessary to evaluate the state of cardio-vascular state of the patient before administration of a drug against erectile dysfunction.
- Interface with Other Medications.
If you take some other medications you should necessarily consult with your doctor about Levitra use.
- Overdosing.
Symptoms: aches in loins are possible.
Treatment: symptomatic and supporting therapy. As Vardenafil is concerned with the whites of plasma in high degree and only a small amount of the drug is cleared by kidneys, the efficiency of haemodialysis is unlikely.
There have been cases of intake of a dose of Vardenafil 80 mg. once a day and 40 mg. once a day during more than 4 weeks without developing of serious side effects. At the same time with the use of the drug in a dose of 40 mg. twice a day there are observed frank pains in loins without the signs of toxic performance on the muscle system and on the nervous one.
